CONDITION OF CONCERN

The headlight switch wire may be subject to bending fatigue. Over time, while the turn signal lever is being used, there is a possibility that this may cause the headlight switch wire to break. This could also lead to the intermittent loss of the headlights and could potentially result in total loss of the headlights.





SUBJECT VEHICLES

OWNER NOTIFICATION

Mazda will notify owners of subject vehicles by mail on February 5, 2002.





REIMBURSEMENT PROGRAM





The reimbursement program has been extended through March 2003. Owners of affected vehicles will receive a reimbursement application form shown.





REQUIRED PARTS

Campaign labels are supplied at no charge and are available in 8-1/2" X 11" sheets containing 18 labels. Please use MNET to order labels.
