OWNER NOTIFICATION
Mazda will notify U.S. owners by first class mail beginning January 31, 2005.
PARTS INFORMATION
Mazda will automatically ship parts to dealers for vehicles in dealer inventory (the majority of vehicles in this recall are in dealer inventory). If a part is needed for a customer vehicle, please complete an IPFax form for each VIN and submit to the Corporate Dealer Assistance Group. Parts will be shipped as a CSO order. CSO freight charges will be reimbursed through a sublet on the warranty claim
NOTE:
Mazda has parts inventory at PDCs to cover 100% of affected vehicles. Due to the small number of affected vehicles, and to guarantee availability to all dealers and customers when a part is needed, stocking of extra parts at the dealerships will not be possible
Use MStore to order additional Campaign labels (1=sheet of 18 labels).

RENTAL CAR
A rental car may be provided to the customer if eligible based on the terms and conditions of the Rental Car Reimbursement Program, policy 12.0. Rental car reimbursements are available only on 2001 and newer vehicles within the mileage and time limitations under the New Vehicle Limited Warranty. If the customer was placed in a rental car while the campaign was being completed and the vehicle is not within the mileage and time limitations, you must obtain authorization for the rental.
Submit a separate claim/problem using the standard rental claim information.
