NOTICE REMOVED, EXTERNALLY CRACKED CYLINDER HEAD

WIRE TAG THE HEAD - INCLUDE YOUR P&A CODE, VIN NUMBER AND "RECALL 89S66". ATTACH A COMPLETED COPY OF THE FORM 1864 (ALONG WITH THE TAG) TO THE HEAD, AND HOLD THE HEAD FOR DISTRICT FIELD SERVICE ENGINEER INSPECTION.

Parts will not be direct shipped for this recall.

Your cylinder head parts order must contain:

- Name of contact at dealership
- Description/location of crack
- VIN number
- P & A Code
- Shipping address
- 1864 Claim number

Prior district approval is required for Claim Form 1864 submission. The Field Service Engineer must subsequently review the removed cylinder head assembly which has been tagged and held by dealer according to the above notice.

Your dealership will be billed the full dealer price of the head including air freight shipping cost if the removed head does not have an external crack.
