Claims Preparation Instructions

Repairs provided under this program will be performed at no charge to owners for 60 months from the vehicle in-service date regardless of distance travelled or through October 31, 1988 whichever provides the greater coverage. Repairs required after this limitation will not be covered under this program but will be the owners's responsibility.

Claims Forms 1864 for this program must include standard claim preparation procedures showing all parts used, their cost and 30% parts handling allowance. Claims must show Program Number M64, be imprinted, dated and signed by authorized dealership personnel.

Enter the completed 1864 claim through the Direct Warranty Entry (DWE) system. Should you receive 1864 forms with a seven digit claims number, drop the first digit and enter only the LAST SIX digits. You must be sure the claim number is correct when entered. Dealers without DWE capability must mail the second copy of the Form 1864 for reimbursement. Claims must be submitted for all completed units.
