Odometer/Circuit Board Ordering Procedure

Make a photocopy of the Replacement Odometer Order Form (see Figure 1).

Then fill out the form and submit it as follows.

1. Enter the current mileage on the vehicle's existing odometer.

2. Enter the mileage at the time of the previous odometer replacement.

Obtain this mileage figure from the VIN List.

3. Fill out the remainder of the order form.

4. Fax the order form to Model Electronics Inc. at (845) 356-9641.

NOTES:

^ No phone orders will be accepted.

^ A new odometer/circuit board will be sent to you via Fed-Ex overnight mail.

5. Once faxed, attach the order form to the repair order.
