Warranty claims should be submitted by using the following information through DCS (Independent Distributor Dealers - Follow Established Procedures).

Warranty Code: R (NOTE: use separate claim for recalls)



1. INSPECTION ONLY

Damage Code: 427499F04 8* for inspection of both spring plates

(*note new disposition code without parts replacement)

Labor Operation: 42741208 for cleaning, testing and inspection of both spring plates

Time Units: 50TU

2. VEHICLE AND REPLACEMENT OF ONE SPRING PLATE

Damage Code: 427499F04 2 for inspection of both spring plates and replacement of one side

Labor Operation: 42741218 for cleaning, testing, and inspection of both sides, and replacement of one spring plate

Time Units: 660TU

Part Number: Either 00004303900 Left side kit (includes all required parts) or 00004304000 Right side kit (inlcudes all required parts)

Parts $ Amount: $69.54 (see attached claim sample)




3. VEHICLE INSPECTION AND REPLACEMENT OF BOTH SPRING PLATES

Damage Code: 427499F04 2 for inspection of both spring plates and replacement of both sides

Labor Operations: 42741228 for cleaning, testing, inspection and replacement of both sides

In cases where the recall cannot be performed, submit the information on a claim via DCS (Independent Distributor Dealers - Follow Established Procedures) with the appropriate disposition code by inserting the code in at the last digit of the damage.

Owner Name and Address Change

If owner name and address is shown incorrectly, submit correction using name/address change postcard from Warranty and Maintenance booklet or recall mailing, or report to PCNA via DCS on the warranty "Recall Owner Update" function.

Parts Retention

Replaced parts must be held and scrapped by your Field Service Specialist.
